Custom Chart Tray Order Form

A.  Please describe your location in as much detail as possible in the form below, and/or 

B. Use the google map to identify your desired map location.

  1. Drag the map to your desired location so that key landmarks/bodies of water are included in the map box.

  2. Zoom in/out so that there is a little extra space on the map around the area you would like to have captured on your Nautical Chart Tray.

  3. Screenshot the map box. On Mac —> Command + Shift + 5. On Windows —> Windows button + Shift + S.

  4. Attach the screenshot of the map box to the form below. Davita will reach out within the next several days to ensure the final map that we include in your Nautical Chart Tray aligns with your vision.

  5. Proceed to the “Shop” page after submitting the form below and select “Custom Chart Tray” to complete checkout and pay for your order.

Please Note: your Chart Tray will include a Nautical style chart of the selected area. The Google Map preview here is used to help confirm location and key landmarks to include. Davita will send you a preview of the Nautical style chart to confirm it aligns with your vision.
